Mental Health Key Worker

Job Description

This position is for a Casual Mental Health Key worker at Primacy Care Australia, a growing NDIS service provider. We are seeking energetic, passionate, and highly committed individuals to be part of our growing team. In this position, you will be responsible for helping individuals living with psychosocial disability, participate and engage in social and community activities. You will be supporting NDIS participants with empathy, humanity, and care. Primacy Care Australia works on a Key worker model meaning you have own case load to perform autonomously.

Job Description

  • Fortnightly care meeting which allows you to build connection with whole team.
  • On the job training and mentoring.
  • You will be allocated to a Service delivery Coordinator which you will have access daily to debrief and mentored.
  • Work on a Key worker model / Case Manager with own case load.

Responsibilities and Duties

  • Provide support to participants in line with Primacy Care's social model of recovery.
  • Offer support to participants and assist them to participate in all lifestyle activities of their choice and meet daily living needs as well as attain better social participation in their community.
  • Provide short-term focused interventions, psychoeducation, support and mentoring to participants.
  • Support and assist participants to achieve their goals and needs in relation to approved NDIS plan.
  • Deliver high quality, person-centred services to meet the needs of participants.
  • Provide weekday and weekend support as required by participants.

Qualifications and Skills

To be eligible for this position, you must:

  • Available for minimum of 5 days a week.
  • Have a full Australian Driver’s License.
  • Current Working with Children's Check.
  • Current valid First Aid Certificate.
  • Current National Police Check.
  • Own your own car and have full comprehensive roadworthy vehicle insurance.
  • An understanding of the National Disability Insurance Scheme (NDIS) and the nature of psychosocial distress.
  • An understanding on how to manage a case load and do this autonomously.
  • An ability to organise your own timetable/roster and meet deadlines/appointments.
  • An ability to write comprehensive case notes and do this articulately, clearly, and concisely.
  • A passion for mental health and have high work ethics
  • Personal / Lived experience in the mental health will be highly desirable.
